ATTENTION!

You no longer need to submit the registration form, the player's photo (when required) nor the player's birth certificate via the mail! That's right, nothing additional to do after finishing here online! AYSL will now generate your registration form for you and you will just have to sign it at your player's first practice! That simple! If your child is in an age group that requires a photo, we now offer you the ability to upload a photo during the registration process! 

If you have not submitted a proof of age document for your child, it is now required to complete your registration. You will need to either upload it during registration, e-mail it, or send via the US Mail. Failure to do so will delay your child's registration. Instructions for submittal are included in the registration process - if your child's document needs to be submitted.
